How to Apply
Fill out the IMAP entry form . Please print or type. Use additional pages as needed.

On a separate sheet, please provide detailed information on each aspect of your advocacy efforts as outlined below:

  1. Explain your advocacy efforts and be specific in terms of your goals, partnerships, target audience and collaborations that helped you meet your objectives.
  2. Tell us about your chapter or institute, your volunteer leadership or staff. What is your geographic area like, your budget and what constraints did you have (if any) with your initiatives? Any statistics, benchmarks or anecdotal information is helpful.
  3. Describe the needs your campaign filled and detail your long-term strategy for success.
  4. Explain how you measured the impact of your efforts on your organization and the profession.
  5. Explain why your advocacy strategies should receive the IMAP Award.
  6. Attach samples and supporting documentation (articles, clippings, photographs, newsletters, Web pages, promotional materials, etc.).

NOTE: COPIES OF ALL SUPPORTING DOCUMENTATION MUST ACCOMPANY THE ENTRY FORM. Although the program description and entry form must be submitted in English, clippings, samples, and other attachments may be submitted in their original form and language.
